Effective Treatment of Sepsis patients from hospitalization through the post acute phase : Is it possible to review existing evidence and come up with a set of guidelines that will guide clinicians in caring for Sepsis patients during and after hospitalization? What strategies are recommended? Focus is on the adult population.
Describe why this topic is important.
The mortality rate of sepsis patients is on the rise despite the guidelines released by the society of critical care medicine several years ago. Existing guidelines tend to be focused on the early interventions with little reference on how to continue care once early resuscitation is completed.
Tell us why you are suggesting this topic.
Our hospitals are looking to decrease the sepsis mortality rates in our adult inpatients. What we notice is that the clinicians have mixed opinions on whether the sepsis bundle interventions that are promoted by CMS really make a difference. Some argue that they do not believe the aggressive intravenous fluid administration is necessary. Also there are no good guidelines about how to manage the patient once they leave the hospital and manage those with post-sepsis syndrome. There is also a lot of confusion around proper coding guidelines for sepsis patients.

Describe what you are doing currently and what you are hoping will change because of a new evidence report.
We have sepsis committees at each of our facilities that review the sepsis mortalities and work with clinicians to promote use of the existing sepsis bundle as well as educate physicians on coding guidelines for sepsis patients.
How will you or your group use the information from a new evidence report?
We will use the evidence given to guide the work of our sepsis mortality committees and hopefully shape clinical practice to lower our sepsis mortality rates.
How would you or your group plan to disseminate information from the report? Who would you plan to disseminate it to?
We have 4 acute care hospitals involved and we will make sure that the information is shared at all entity committees and the medical staff in those entities.
Do you know of organizations that could use an evidence report to change clinical practice? Are you a part of, or have you been in contact with, any organizations that might implement the research findings of an evidence report?
The California Hospitals association has a Quality improvement division - HQI that would be a great organization to disseminate the information to other California hospitals. We have not approached them on this subject.
