Palliative Care for Multiple Chronic Conditions

1. Why are you struggling with this issue?

We would like to evaluate how implementing effective symptom management (palliative care) in symptomatic multiple chronic conditions can reduce disease exacerbation, prevent hospitalization, debility and maintain physical functioning.

2. What do you want to see changed? How will you know that your issue is improving or has been addressed?

Attention to effective symptom management or effective integration of palliative care can reduce disease exacerbation in the nation’s largest patient population - those living with 2 or more chronic conditions. Palliative interventions are not associated with serious illness, end-of-life or hospice care.

Support a recent consensus statement proposal currently under review with the American Academy of Nursing: Implementing palliative care in the effective management of multiple chronic conditions
