What are the benefits and harms of digoxin in the treatment of atrial fibrillation and/or heart failure in older adults? Are the benefits and harms dose dependent, i.e., do they differ for doses less than or equal to 0.125 mg per day compared to 125 mg per day?

Describe why this topic is important.

Regarding digoxin, the pendulum has swung back and forth in the literature. Currently digoxin is listed as avoid as first-line therapy for both conditions in the 2015 AGS Beers Criteria and clinical practice guidelines.

How will an answer to your research question be used or help inform decisions for you or your group?

It will help the AGS Beers Criteria panel decide shape its recommendation for both of these criterion in the 2018 AGS Beers Criteria, particularly if the potential for harm is substantially greater than the expected benefits in a subgroup of older adults, e.g., residents of long term care, ambulatory community-dwelling elderly.
