What is the value of predictive analytics? Particularly in population health. Government had created payment systems that reward or punish for readmission rates. Managed through predictive analytics that calculate risk scores for risk of rehospitalization. What are components of good predictive risk score? are there better predictive analytical for hot spotters/high utilizers. How orient through less expensive and more effective resources

Some health systems are incentivized by the CMS initiative. They would be interest in a compilation of evidence on the predictive value of validated instruments that can identify patients at high risk of readmission. Once identified accurately patients who are at high risk of rehospitalization. The next step would be to direct greater effort and care towards these patients to reduce that risk of 30-day hospital readmission.
