ACC/AHA would like to suggest updating already existent AHRQ systematic reviews with the questions and evidence gaps addressed therein, published in 2013.

Describe why this topic is important.

Stroke Prevention in Atrial Fibrillation:

  1. With regards to predicting thromboembolic and bleeding risk: The available studies for the scores had methodological issues so further studies were needed to utilize complete data, use validated outcomes and compare scores using appropriate statistical methods.
  2. With regards to interventions for preventing thromboembolic events: A better understanding of comparative safety and effectiveness of newer anticoagulant therapies is needed.
  3. With regards to anticoagulation strategies in patients undergoing invasive procedures: RCTs were needed with traditional and newer anticoagulants to determine optimal protocols for anticoagulation either during radiofrequency ablation or other operative procedures.
  4. With regards to switching between Warfarin and novel oral anticoagulants and stroke prevention after a hemorrhagic event: There were no peer review publication to address these questions so an update can be helpful.

ACC/AHA writing committees use evidence-based methodologies to search the literature and formulate recommendations. Independent and quality systematic reviews by organizations such as AHRQ help inform the guideline writing committees to frame trust worthy guidelines.
