Nursing Facilities, versus Community-based care vis HCBW and PACE

NF facility eligibles > 55, dually eligible and Medicaid only.

Multi system complex disease. including dementias

Describe the health-related benefits you are interested in. (For example, improvements in patient symptoms or problems from treatment or diagnosis.)

Maximum functionality, decrease in unecesary hospitalizations, ER utilization, adequate preventitive care, effective advanced directives.
