Describe why this topic is important.

For the treatment of open-angle glaucoma, it is desirable to have a lowering of intraocular pressure throughout day and at night. It is not clear whether or not laser surgery as the primary treatment can lower intraocular pressure at night and how well when compared to the use of medications.

What specifically motivated you to ask this question? (For example, you are developing a clinical guideline, working with a policy with large uncertainty about the appropriate approach, costly intervention, new research you have read, items in the media you may have seen, a clinical practice dilemma you know of, etc.)

The preferred practice pattern in treating open-angle glaucoma in the United Sates is to start with a medication. It may cost more and have more side effects after long-term use, compared to laser surgery. However, the effectiveness of laser surgery as a primary treatment at night is unclear.
