Non-metastatic Gastric Cancer Management

1. What is the decision or change (e.g., clinical topic, practice guideline, system design, delivery of care) you are facing or struggling with where a summary of the evidence would be helpful?

Gastric cancer is the fifth most common cancer worldwide, and most frequent cause of cancer death in some countries. We are interested in the perioperative management of non-metastatic gastric cancer. This topic would include indications for endoscopic ultrasound, diagnostic laparoscopy, perioperative chemotherapy, adjuvant chemotherapy, and the role of radiation.

2. Why are you struggling with this issue?

There is significant controversy regarding optimal management of non-metastatic gastric cancer. For example, perioperative chemotherapy and diagnostic laparoscopy may be underutilized. Our goal would be to use this review as the basis for a clinical practice guideline that addresses the uncertainties, and aids in reducing practice variation in the treatment of this disease site.

3. What do you want to see changed? How will you know that your issue is improving or has been addressed?

We want to see a reduction in practice variation and a raised awareness of the options for treatment of non-metastatic gastric cancer. We also would like to use the evidence review to highlight areas of uncertainty for which more research is needed. We will create accompanying tools and resources when our associated guideline is complete in order to aid in implementation and will monitor downloads of the resulting clinical practice guideline.

5. What will you do with the evidence report?

We would develop an associated clinical practice guideline with a panel of clinical experts. The goal would be to give clinicians, particularly medical oncologists in community practice, evidence-based guidance for the treatment of this disease site. This topic is one of the American Society of Clinical Oncology priorities for 2023, but we are flexible on timing if AHRQ is willing to take on this topic.
