The approparite medical management, patient education, pre-discharge teaching; post discharge follow up.

Prevent re-admissions and improve quality of life and possible early/pre-mature death due to poor aherence to diet, etc.

Prevent early death. Also re-admissions within 30 days of discharge for the same DRG are not reimbursable, therefore we lose $$$.

What specifically motivated you to ask this question? (For example, you are developing a clinical guideline, working with a policy with large uncertainty about the appropriate approach, costly intervention, new research you have read, items in the media you may have seen, a clinical practice dilemma you know of, etc.)

Sad to say the emphasis on preventing the re0admissions was primarily brought about due to financial impact. So, clinical management needs to improve to reduce financail strain.
