Medication Adherence measures—what are the best practices to help patients and improve outcomes for these measures? Examples of medication adherence measures include and not limited to: Asthma Medication Use, Statin use in cardiovascular diseases or diabetes, proportion of days covered for various chronic disease states.

2. Why are you struggling with this issue?

There are data issues that affect the accuracy of these measures. For example, not all the dispensed pharmacy events are captured, since the data collection depends on claims data collected by the payer.  if the patient self pays, the payer will not have a claim. There are cases where patients split pills for cost reasons and they may show up as non-adherent. Health systems can benefit from learning what interventions are most successful to improve medication adherence and do the approaches vary by disease condition. How do provider engagement tactics compare with patient engagement approaches to improve medication adherence measures? What are the best patient education and clinician engagement materials and scripts that has shown to be most effective for these measures?

3. What do you want to see changed? How will you know that your issue is improving or has been addressed?

Identify a library of evidence based approaches that health systems and clinicians can use to improve medication adherence.
