Social Determinants for Hospitalized Youth

Topic Suggestion

1. What is the decision or change you are facing or struggling with where a summary of the evidence would be helpful?

What is the effectiveness of screening for social determinants of health for hospitalized infants and children and their families for long term health and well being.

2. Why are you struggling with this issue?

Although it is now accepted that the social determinants of health are as if not more important in determining the long term health and well being of hospitalized infants, children and their families as the medical interventions provided, there is uncertainty about benefits and appropriate tools for screening for social determinants with substantial variation in practice.

3. What do you want to see changed? How will you know that your issue is improving or has been addressed?

That health professionals recognize that their responsibility to patients and families extends beyond the hospital walls and that there are effective interventions that they can implement to address these issues particularly for patients who are suffering from the injustices of racism, segregation, inequality of income, wealth and opportunity and xenophobia. We will know the issue is improving when clinical teams recognize their responsibility to practice social as well as technical medicine.

5. What will you do with the evidence report?

The Vermont Oxford Network will disseminate the findings widely in our worldwide community of practice (over 1200 hospitals providing newborn and pediatric care) and assist teams to incorporate the evidence based practices in our quality improvement collaboratives.
